PLEASE NOTE: The bivalirudin dosing for heparin induced thrombocytopenia treatment is significantly different than the dosing used in the Cath Lab. Please encourage the use of the protocol PowerPlans “HOSP BIVALIRUDIN (ANGIOMAX) INFUSION - LOW SLIDING SCALEor “HOSP BIVALIRUDIN (ANGIOMAX) INFUSION - Standard SLIDING SCALE” whenever bivalirudin is being used to treat heparin induced thrombocytopenia.
