  • 0.3 - 1.5 mg/kg/day

amphotericin B liposomal (AmBisome)

  • 3 - 5 mg/kg

Discuss interchange with prescriber as conversion is not 1:1. Fungizone use restricted to neonates, not routinely stocked.  Suggest use of AmBisome until Fungizone purchased.
