  • Tacrolimus capsules and granules have equivalent dosing. Tacrolimus granules in unit dose packets cannot be divided.
  • Other brands of tacrolimus or generic tacrolimus cannot be mixed or interchanged with ENVARSUS XR.
    • Conversion from immediate-release tacrolimus to ENVARSUS XR should be done under the supervision of the transplant team.
  • When converting between formulations:
    • The first dose of ENVARSUS XR should be given the morning after the last evening dose of an IR tacrolimus product

*Brand Prograf capsules are non-formulary and may only be used in patients as continuation of home therapy.
