Tacrolimus capsules and granules have equivalent dosing. Tacrolimus granules in unit dose packets cannot be divided.
Other brands of tacrolimus or generic tacrolimus cannot be mixed or interchanged with ENVARSUS XR.
- Conversion from immediate-release tacrolimus to ENVARSUS XR should be done under the supervision of the transplant team.
When converting between formulations:
- The first dose of ENVARSUS XR should be given the morning after the last evening dose of an IR tacrolimus product

*Brand Prograf capsules are non-formulary and may only be used in patients as continuation of home therapy.
