ISMP recommended Tall Man Letters vinCRIStine to avoid mistaken administration of vinBLAStine.

 

Caution: FATAL IF GIVEN INTRATHECALLY. FOR INTRAVENOUS USE ONLY. To minimize the risk of inadvertent intrathecal vincristine administration, it is Genesis Medical Center practice to dilute all intravenous vincristine doses in a 50 ml minibag prior to dispensing.

This is a high risk medication. Double checks for the appropriateness of drug, dose, concentration, and pump setting (if applicable) must be performed.
