*Please note: 4F-PCC contains heparin and should not be used in patients with a history of HIT*

For warfarin reversal: 
- Per policy, standard dose of 1500 units will be used
- Vitamin K 10 mg IV x 1 should be administered concurrently
- Provider may order an extra 4F-PCC 500 units if deemed clinically necessary after first administration of 4F-PCC 1500 units and Vitamin K 10 mg

For oral factor Xa reversal (i.e. apixaban, betrixaban, edoxaban, rivaroxaban):
- Per policy, standard dose of 2000 units will be used
- Dose may be repeated if necessary, based on patient response
