ISMP recommended Tall Man Letters: oxyCODONE (generic) and OxyCONTIN (extended release brand).  "IR" and "CR" should be used to differentiate products.

 Do not crush extended release tablets (Oxycontin). Doing so may cause a potentially fatal overdose.
