Dispense a Humalog 3 ml Kwikpen.   Therapeutic Substitution of Insulin

Nonformulary due to no clear advantage over insulin lispro and may increase potential for sound alike/look alike insulin medication errors. 

This is a high risk medication. Double checks for the appropriateness of drug, dose, concentration must be performed.

ISMP recommends Tall Man Letters NovoLOG to avoid mixups with NovoLIN.

Dispose of in black pharmaceutical container (contains creosol)
