ISMP recommended Tall Man Letters hydrOXYzine. Confusion between hydroxyzine and antihypertensive agent hydralazine could lead to serious adverse drug events.

The High Risk/High Alert Medication Policy may be found by using the search function in Policies and Procedures.

Hydroxyzine Pamoate is now nonformulary, and hydroxyzine HCl will be substituted, same dose and frequency.
