Comments:

Inform prescriber that this product is a non-formulary item and recommend ceftazidime and the addition of vancomycin IV per pharmacy dosing if MRSA coverage is desired. If DAW, an ID consult must be obtained by the prescribing physician and an order received from an ID provider prior to dispensing.

Ceftazidime dosing recommendations:

	Greater than 80 kg	Less than or equal to 80 kg
CrCl greater than 50 ml/min	Ceftazidime 2 gm IV q 8 hours	Ceftazidime 1 gm IV q 8 hours
CrCl less than or equal to 50 ml/min	Adjust dose and frequency per product information for renal dysfunction
