Comments:

Ipratropium/Albuterol (Duoneb) 0.5 mg/2.5 mg q 6 hours, is to be automatically substituted for the MDI except when criteria specified by P&T is met. If the patient meets P&T criteria regarding nonsubstitution, the MDI is to be substituted with Anoro Ellipta (Umeclidinium/Vilanterol). See the therapeutic interchange table for specifics.

Intubated patients are excluded from this substitution. This substitution does not apply to PRN orders.
