Upadacitinib is a RESTRICTED formulary medication. Use is restricted to patients > 12 yo and > 40 kg who have failed indicated medication therapy and are only left with surgical intervention treatment options for IBD. Must be able to swallow tablet whole. Medication will not be routinely stocked and will only be procured pursuant to active orders on patient who meets above restriction. All other use requires approval by Pharmacy Leadership.
