Siltuximab is a RESTRICTED formulary medication. Use is restricted to patients with Castleman Disease after insurance approval in an outpatient infusion clinic or pursuant to the Kymriah protocol for patients who fail Tocilizumab. Siltuximab will only be stocked pursuant to an active patient and ordering is restricted to hem/onc prescribers and the dual signature oncology process.
