Siltuximab is a RESTRICTED formulary medication. Use is restricted to patients with Castleman Disease after insurance approval in an outpatient infusion clinic or for patients who experience CRS unresponsive to Tocilizumab or who experience ICANS unresponsive to Steroids. A 400 mg dose of Siltuximab will be stocked by pharmacy.
