Meropenem is a RESTRICTED formulary medication. Use is restricted to patients who meet the below criteria:
COA approved indications:
*ALL use requires ID consult PRIOR to use even for "approved" indications unless patient is in the ICU. In the ICU an ID consult must be obtained within 72 hours.*
Culture/Sensitivity documenting ESBL infections
H/O or current infection with organism that is confirmed resistant to at a minimum cefepime and/or zosyn
Gram negative infection involving the CNS (Pseudomonas, Enterobacter, Klebsiella)
Febrile neutropenic patient meeting the following criteria: Fever > 72 hours + hemodynamic instability and/or resp distress despite adequate GNR coverage requiring transfer to higher level of care AND abdominal symptoms with an allergy/intolerance to Zosyn
Sepsis with no source meeting the following criteria: H/O ESBL OR abdominal sepsis with prolonged exposure to cefepime or zosyn for > 7 days
