Linezolid (Zyvoxx) is a RESTRICTED formulary medication.  Use of this medication requires approval from an ID attending or fellow and an ID consult.

Use is restricted to patients who meet one of the below criteria:

• Culture positive or history of VRE infections
• Persistent VRE meningitis (to be used in combination with daptomycin)
• Persistent MRSA bacteremia in a patient with a vancomycin allergy and failure to alternative treatment option
