Leuprolide injection is a RESTRICTED formulary item. Use is restricted to outpatient clinics. Only the 5mg/ml vial will be kept in stock in the pharmacy. All pre-filled syringes will be ordered pursant to a scheduled patient visit and require pharmacy notification of dose to be ordered at least 72 hours prior to the appointment.
