Cidofovir is a RESTRICTED formulary medication. Use is restricted to patients with the following diagnosis after ID approval:
- Adenovirus- BK virus- CMV resistant or refractory to standard therapy- HSV resistant or refractory to standard therapy
All other uses require approval from Antimicrobial Stewardship and/or pharmacy leadership.
