Ordering of Ceftazidime for intravitreal or ophthalmic administration is RESTRICTED to the OS for compounded ophthalmic formulations using clinical decision support. Only the 2 mg/0.1 ml concentration for intravitreal use and the 50 mg/ml concentration for ophthalmic use that have been approved are allowed for use.
