Cangrelor is a RESTRICTED formulary medication. Use of Cangrelor is restricted to: - patients who are on oral antiplatelet and have a scheduled procedure in the next 4 days necessitating holding of the oral medication. - patients needing antiplatelet therapy who are NPO for all medications. - patients needing antiplatelet therapy who are not achieving adequate function with clopidigrel, as indicated by Platelet Reaction Units (PRU) greater than 180 using the “VerifyNow P2Y12 monitoring system”.
- patients requiring emergent antiplatelet therapy during IR procedures within the OR
All other use of Cangrelor requires approval by Pharmacy Leadership.
