System Formulary Update
UTI Agents

Situation
The UTI agent class review and formulary standardization was approved by the System Pharmacy and Therapeutics Committee in February 2020.

Background
The following medications were reviewed: fosfomycin, methenamine, nitrofurantoin, nitrofurantoin monohydrate, trimethoprim

Assessment/Recommendation
Changes Effective: Tuesday, October 13, 2020

System P&T voted to include the following products on the UNC Health Medication Formulary: 

• Fosfomycin 3-gm packet 
• Nitrofurantoin monohydrate macrocrystal 100-mg SR cap 
• Nitrofurantoin 25 mg/5 mL oral suspension 
• Trimethoprim 100-mg tablet 
• Trimethoprim 50 mg/5 mL oral suspension 

Note: Stock of these products may vary at individual entities

As a result, the following products will be removed from the UNC Health Medication Formulary: 

• Methenamine hippurate 1-gm tablets and methenamine combination products
• Nitrofurantoin macrocrystal 25-mg capsule 
• Nitrofurantoin macrocrystal 50-mg capsule 
• Nitrofurantoin macrocrystal 100-mg capsules
