Situation/Background
Bacitracin ophthalmic ointment 3.5 g is no longer commercially available and is being removed from the UNC Health System Formulary. This was approved at the December 2021 System Pharmacy and Therapeutics Committee meeting.

The following products will be removed from the inpatient drug formulary:
- Bacitracin 3.5 g ophthalmic ointment (ERX 852; E289)
