System Formulary Update
Vitamin B Products

Situation

At the November 2021 System Pharmacy and Therapeutics (P&T) Committee meeting thiamine mononitrate tablet was added to System Formulary and thiamine HCl tablet was removed from System Formulary.

Background

Originally, System P&T Committee approved to standardize vitamin B analogs in April 2019 with an amendment to exclude additional vitamin B complex products in November 2019.

In November 2021, the specific vitamin B complex combination medications reviewed were thiamine mononitrate tablet and thiamine HCl tablet.

Assessment/Recommendation

Changes effective: Tuesday, December 21

System P&T voted to include the following products on the UNC Health Care System Drug Formulary:

• Thiamine mononitrate tablet
• Thiamine injection
• Riboflavin tablet
• Pyridoxine tablet, injection
• Biotin oral suspension
• Folic acid tablet, injection, oral solution
• Cyanocobalamin tablet, injection
• Hydroxycobalamin injection
• Vitamin B complex tablet

As a result, the following products will be removed from the inpatient drug formulary:

• Thiamine HCl tablet
• Niacin
• Niacinamide
• Pantothenic acid
• Biotin tablet, capsule
• Other Vitamin B complex products (includes Nephron FA and Folbic)

Non-formulary orders will be converted to formulary products via to the following therapeutic interchange:
