Antipsychotics, First Generation Formulary Update

Situation

The antipsychotics, first generation class review and formulary standardization was approved at the September 2021 System Pharmacy and Therapeutics Committee meeting.

Background

The following medications were reviewed: chlorpromazine, droperidol, fluphenazine, haloperidol, loxapine, molindone, perphenazine, pimozide, prochlorperazine, thioridazine, thiothixene, trifluoperazine

Assessment/Recommendation

Changes effective: Tuesday, November 16, 2021

System P&T voted to include the following products on the UNC Health Medication Formulary:

  • Chlorpromazine injection and tablet
  • Droperidol injection
  • Fluphenazine injection (deaconate and HCl) and tablet
  • Haloperidol injection (deaconate and lactate), oral solution, and tablet
  • Loxapine capsule
  • Perphenazine tablet
  • Prochlorperazine injection and tablet
  • Thiothixene capsule
  • Trifluoperazine tablet

Note: Stock of these formulary products may vary at individual entities

As a result, the following products will be removed from the UNC Health Medication Formulary:

  • Fluphenazine oral elixir and oral concentrate solution
  • Loxapine aerosol inhalation
  • Molindone
  • Pimozide
  • Prochlorperazine rectal suppository
  • Thioridazine

Non-formulary orders will be converted according to the following therapeutic interchange:

Amitriptyline-perphenazine combination product

Equivalent dose of amitriptyline + perphenazine
