Oxacillin Addition/Nafcillin Removal Formulary Update

Situation

Oxacillin was approved for addition to the UNC Health system formulary by the System Pharmacy and Therapeutics Committee. Nafcillin was approved for removal from UNC Health system formulary at this same meeting.

Background

The following medications were reviewed: Oxacillin 1 g, 2 g, and 10 g vials; Nafcillin 1 g, 2 g, and 10 g vials.

Assessment/Recommendation

Change effective: Tuesday, June 30th         

System P&T voted to include the following products on the UNC Health Medication Formulary:

• Oxacillin 1 g, 2 g, and 10 g vials

System P&T voted to exclude the following products from the UNC Health Medication Formulary:

• Nafcillin 1 g, 2 g, and 10 g vials
