Koate & Hemofil-M (Factor VIII) Formulary Update 

Situation 

Koate (Factor VIII) 500 IU & 1,000 IU vials were approved for addition to the health system formulary while Hemofil-M (Factor VIII) vials were approved for removal from the health system formulary at the January 2026 System Pharmacy and Therapeutics Committee meeting.  

Background 

The following medication was reviewed: Koate (Factor VIII) 500 IU & 1,000 IU vials, Hemofil-M (Factor VIII) 

Assessment/Recommendation 

Change effective: March 3, 2026        

System P&T voted to include the following products on the UNC Health Medication Formulary: 

Koate (Factor VIII) 500 IU & 1,000 IU vials  

System P&T voted to exclude the following products on the UNC Health Medication Formulary: 

Hemofil-M (Factor VIII) vials
