Sclerosing Agents Formulary Update

Situation

The sclerosing agent class review and formulary standardization was approved at the November 2025 System Pharmacy and Therapeutics Committee meeting.

Background

The following medications were reviewed:

• Ethanol 99% (5 mL) injection (E12758/ERX 186326)
• Ethanolamine oleate 5% (2 mL) injection (E6175/ERX 9984)
• Polidocanol 1% (20 mg/2 mL) injection (E5958/ERX 112481)
• Polidocanol 1% (18 mL) foam injection (E10632/ERX 172066)
• Sodium tetradecyl sulfate 1% (2 mL) injection (E3177/ERX 41787)
• Sodium tetradecyl sulfate 3% injection (E3178/ERX 41793)
• Sterile talc (Steritalc) aerosol powder 3 g (E12892/ERX 184311)
• Sterile talc (Steritalc) powder 4 g (E10062, E15244/ERX 40842288, ERX 184312)
• Polidocanol 0.5% injection 10 mg/2 mL (no E#)
• Sterile talc (Steritalc) powder 2 g (no E#)
• Sterile talc (Sclerosol) intrapleural aerosol powder 4 g (ERX 32569)
• Sterile talc (Steritalc) powder 5 g (E7040)

Assessment/Recommendation

Changes effective: Tuesday, December 9, 2025

System P&T voted to include the following products on the UNC Health Medication Formulary:

• Ethanol 99% (5 mL) injection (E12758/ERX 186326)
• Ethanolamine oleate 5% (2 mL) injection (E6175/ERX 9984)
• Polidocanol 1% (20 mg/2 mL) injection (E5958/ERX 112481)
• Polidocanol 1% (18 mL) foam injection (E10632/ERX 172066)
• Sodium tetradecyl sulfate 1% (2 mL) injection (E3177/ERX 41787)
• Sodium tetradecyl sulfate 3% injection (E3178/ERX 41793)
• Sterile talc (Steritalc) aerosol powder 3 g (E12892/ERX 184311)
• Sterile talc (Steritalc) powder 4 g (E10062, E15244/ERX 40842288, ERX 184312)

Note: Stock of these formulary products may vary at individual entities

As a result, the following products will be removed from the UNC Health Medication Formulary
