Situation 

Midazolam in sodium chloride PF (50 mg/50 ml) (STAQ, API) was approved for addition to the UNC Health system formulary at the August 2025 System Pharmacy and Therapeutics Committee meeting.  

Background 

The following medication was reviewed: Midazolam in sodium chloride PF (50 mg/50 ml) (STAQ, API) 

Assessment/Recommendation 

Change effective: Tuesday, September 30 2025 

System P&T voted to include the following products on the UNC Health Medication Formulary: Midazolam in sodium chloride PF (50 mg/50 ml) (STAQ, API).
