Situation: Fentanyl 10 mcg/mL 2 mL syringes PF (STAQ, API) was approved for addition to the UNC Health system formulary at the August 2025 System Pharmacy and Therapeutics Committee meeting.  

Background: The following medication was reviewed: Fentanyl 10 mcg/mL 2 mL syringes PF (STAQ, API) 

Assessment/Recommendation 

Change effective: Tuesday, September 30 2025 

 System P&T voted to include the following products on the UNC Health Medication Formulary: Fentanyl 10 mcg/mL 2 mL syringes PF (STAQ, API).
