System Formulary Update
Ponatinib (Iclusig)

Situation: Ponatinib (Iclusig) was approved for addition to the UNC Health System Formulary at the System Pharmacy and Therapeutics Committee meeting.

Background: The following medication was reviewed: Ponatinib (Iclusig) 15 mg tablets

Assessment/Recommendation: System P&T voted to include the following product to the UNC Health Medication Formulary with restrictions

• Ponatinib (Iclusig) 15 mg tablets

Inpatient use is restricted to patients who meet one of the following criteria:

• Newly-diagnosed Ph+ ALL
• Newly-diagnosed Ph+ MPAL/CML myeloid blast crisis

Orders must be signed by a Hematology/Oncology Attending Physician

Formulary/Epic Changes will Go-Live on Tuesday, March 25th
