Situation: Potassium phosphates 250mL premixed bag was approved for addition to the UNC Health System Formulary at the System Pharmacy and Therapeutics Committee meeting.

Background: The following medication was reviewed: Potassium phosphates in 0.9% sodium chloride injection, 250mL premixed bag 

Assessment/Recommendation: System P&T voted to include the following product to the UNC Health Medication Formulary with restrictions

• Potassium phosphates in 0.9% sodium chloride injection, premixed 250 mL bag 

Use is restricted to sites without 24-hour pharmacy coverage 

Formulary/Epic Changes will Go-Live on Tuesday, April 1st
