Situation:

Rivaroxaban granules for oral suspension (155 mL) was approved for addition to the health system formulary at the System Pharmacy & Therapeutics Committee meeting.

Background:

The following medication was reviewed: Rivaroxaban granules for oral suspension (155 mL)

Assessment/Recommendations:

System P&T voted to include Rivaroxaban granules for oral suspension (155 mL) to the UNC Health Medication Formular with the following restrictions to patients who meet ALL of the following criteria:

• Pediatric patient
• Doses < 10 mg
