Tab-A-Vite Multivitamin

Situation:

Tab-A-Vite multivitamin with folic acid 400 mcg tablet was approved for addition to the UNC Health System Formulary. The Thera-M multivitamin tablet was approved to be removed from the UNC Health System Formulary at the System Pharmacy and Therapeutics Committee meeting.

Background:

The following medications were reviewed: Tab-A-Vite multivitamin with folic acid 400 mcg tablet

Assessment/Recommendations:

System P&T voted to include the following product from the UNC Health Medication Formulary:

• Tab-A-Vite multivitamin with folic acid 400 mcg tablet

System P&T voted to remove the following product from the UNC Health Medication Formulary:

• Thera-M multivitamin tablet
