Situation:

Palifermin (Kepivance) 5.16 mg SDV for injection was approved for addition to the health system formulary at the January 2024 System Pharmacy and Therapeutics Commitee meeting

Background:

The following medications were reviewed: Palifermin (Kepivance) 5.16 mg SDV for injection and Palifermin (Kepivance) 6.25 mg SDV for injection (no longer available from manufacturer)

Assessment/Recommendations:

System P&T voted to include the following product from the UNC Health Medication Formulary:

• Palifermin (Kepivance) 5.16 mg SDV for injection

System P&T voted to exclude the following products on the UNC Health Care System Drug Formulary:

• Palifermin (Kepivance) 6.25 mg SDV for injection
