Class Standardization: Topical Hemostatics

Situation

The topical Hemostatics class review standardization was approved by System P&T in June 2023.

Background

The following products were reviewed: Recothrom (recombinant thrombin), Thrombin-JMI (bovine thrombin), Floseal (gelatin with recombinant thrombin), Gel-Flow (gelatin with bovine thrombin), Surgiflo (gelatin with human thrombin), Hemoblast (porcine collagen/bovine chondroitin with human thrombin), Tisseel (fibrin sealant), Vistaseal (fibrin sealant), Artiss (specialty fibrin sealant), Tachosil (fibrin sealant patch), Evarrest (fibrin sealant patch).

Assessment/Recommendations

System P&T voted to include the following products on the UNC Health System Drug Formulary:

• Recothrom 5,000 unit kit
• Recothrom 20,000 unit kit
• Thrombin-JMI 5,000 unit kit
• Thrombin-JMI 20,000 unit kit
• Floseal 5 mL kit
• Floseal 10 mL kit
• Tisseel VHSD 2 mL syringe
• Tisseel VHSD 4 mL syringe
• Tisseel VHSD 10 mL syringe
• Tachosil 4.8 cm x 4.8 cm patch
• Tachosil 9.5 cm x 4.8 cm patch
• Evarrest 10.2 cm x 5.1 cm patch
• Artiss 2 mL spray set
• Artiss 4 mL spray set
• Artiss 10 mL spray set

As a result, the following products will be removed from the inpatient drug formulary:

• Gel-Flow 5,000 Unit kit
• Surgiflo 8 mL kit
• Vistaseal 2 mL kit
• Vistaseal 4 mL kit
