Situation

The gentamicin (Gentak) 0.3% ophthalmic ointment has been approved for removal by the System Pharmacy & Therapeutics Committee in September 2022.

Background

The following medications were reviewed: Gentamicin (Gentak) 0.3% Ophthalmic Ointment. 

Assessment/Recommendations

System P&T voted to remove the following products on the UNC Health Care System Drug Formulary:

• Gentamicin (Gentak) 0.3% ophthalmic ointment due to being discontinued by manufacturers 

Formulary/Epic changes will Go-Live on Tuesday, November 15, 2022.
