System Formulary Update
Aminoglycosides

Situation

The Aminoglycosides class review standardization was approved in October 2022.

Background

The following products were reviewed: amikacin injection, amikacin suspension for inhalation, gentamicin injection, neomycin tablet, plazomicin injection, streptomycin injection, tobramycin solution for injection, tobramycin powder for injection, tobramycin solution for inhalation, and tobramycin capsules for inhalation.

Assessment/Recommendations

System P&T voted to include the following products on the UNC Health System Drug Formulary:

  • Amikacin 250 mg/mL injection (2 mL, 4 mL)
  • Gentamicin 10 mg/mL injection (2 mL and extemporaneous)
  • Gentamicin 40 mg/mL injection (2 mL, 20 mL)
  • Gentamicin 1.6 mg/mL injection (50 mL)
  • Neomycin 500 mg tablets
  • Tobramycin 1.2 g injection
  • Tobramycin 10 mg/mL injection (extemporaneous)
  • Tobramycin 40 mg/mL injection (2 mL, 30 mL)
  • Tobramycin 60 mg/mL solution for inhalation (5 mL)

As a result, the following products will be removed from the inpatient drug formulary:

  • Gentamicin 0.8 mg/mL injection (100 ml)
  • Gentamicin 1.2 mg/mL injection (100 mL)
  • Plazomicin 50 mg/mL injection (10 mL)
  • Streptomycin 1 g injection
  • Tobramycin 5 g injection

Formulary/Epic changes will Go-Live on Tuesday, December 13, 2022.
