Oral Anticoagulants

Situation
The oral anticoagulants class review and formulary standardization was approved by the System Pharmacy and Therapeutics Committee in September 2020.

Background
The following medications were reviewed: apixaban, rivaroxaban, dabigatran, edoxaban, betrixaban, warfarin

Assessment/Recommendation
Changes Effective: Tuesday, November 10, 2020

System P&T voted to include the following products on the UNC Health Medication Formulary: 

• Apixaban
• Dabigatran
• Rivaroxaban
• Warfarin

Note: Stock of these products may vary at individual entities

As a result, the following products will be removed from the UNC Health Medication Formulary: 

• Betrixaban
• Edoxaban
