Baclofen 5 mg/mL Oral Suspension

Situation

Extemporaneous baclofen 5 mg/mL oral suspension was approved to be removed, while commercial baclofen 5mg/mL oral suspension (Fleqsuvy) was added to the health system formulary at the August 2022 System Pharmacy and Therapeutics Committee meeting.

Background

The following medication was reviewed: Baclofen 5 mg/mL oral suspension (extemporaneous compound) and baclofen 5 mg/mL oral suspension (Fleqsuvy).

Assessment/Recommendations

System P&T voted to include the following products on the UNC Health Medication Formulary:

  • Baclofen 5 mg/mL oral suspension (Fleqsuvy)

System P&T voted to remove the following products on the UNC Health Medication Formulary:

  • Baclofen 5 mg/mL oral suspension (extemporaneous compound)

Formulary/Epic changes will Go-Live on Tuesday, October 4, 2022.
