Note: Per policy, metronidazole will have an automatic frequency change from q8h to q12h with the exception of Central Nervous System infections and C.difficile infections and bacteremia with B.fragilis. This applies to both IV and PO routes.

Note: use 10 day stop date for oral or IV metroNIDAZOLE given for C.diff infections.

Per policy, these drugs are subject to renal dosing adjustments by pharmacy.
