• Administered via IV pump; Not to be given by IV push for any indication.
• Electronic order entries are double checked by another pharmacist or RN.
• U-500 insulin is stored in the pharmacy and dispensed as an insulin pen.
• Patient specific doses shall be stored in lock box in medication room of nursing unit.
• Documentation log is completed by pharmacist.
• Patients with orders for U-500 insulin shall have their dose converted to U-100 insulin per policy.
