 • Electronic order sets available for ordering.
 • Bleeding Precautions sign hung (in Critical Care).
 • Physician/Pharmacist independently verify calculation of dose prior to administration.
 • Two RNs independently verify dosage and pump settings prior to administration.
Per policy, these medications require cardiac and/or special monitoring.
