LA-SA medications are a common cause of medication errors. TJC requires hospitals to maintain a list of commonly confused medications.
Our Unlucky 13 (LA-SA) Lists have been updated for 2022 based on HH medication error reports and events published by ISMP.
Please take a minute to review the LA-SA Action Grid (Actions HH has taken to prevent errors with these medications):
