oxacillin (Bactocill)
nafcillin (Nafcil)
Adults: Interchange at 1:1 ratio.
Pediatric: Convert using dose appropriate for indication and weight; not required to contact MD, but may be necessary to determine indication for use.
oxacillin (Bactocill)
dicloxacillin (Dynapen)
Interchange 250 mg dicloxacillin for 500 mg of oral oxacillin, or proportionally.
Bactocill oral capsule and injection are restricted to NO SUB orders.
