Pharmacists may convert IV to PO on a 1 mg to 1 mg as below:
If insufficient muscle pain or spasm control and if methocarbamol IV was administer for < 3 days:
methocarbamol PO dose may be increased to 1,500 mg q6hr for up to 3 days
and then reduced to 1,500 mg q8hr or 1,000 mg q6hr
