glipiZIDE (Glucotrol) AND metFORMIN (Glucophage)

  • 2.5 mg glipiZIDE tablet PLUS ONE 250 mg metFORMIN
  • 2.5 mg glipiZIDE tablet PLUS ONE 500 mg metFORMIN

metFORMIN -- hold for 48 hours post IV contrast media per radiologist, contraindications with CHF, renal disease, hepatic disease, and age. Pharmacist may order SCr.
