Onfi tablet and oral suspension medication guide
ONFI is approved as a formulary agent. Previously, only the 5 mg tablet was routinely stocked. However, the 5 mg tablet is no longer available and the 10 mg will be stocked in its place (9/3/2013).

clobazam (Onfi)
Sympazan is classified as non-formulary. Patients on Sympazan should be converted to Onfi tablets or suspension, whichever is best appropriate, through a therapeutic interchange at a one-to-one ratio.
Inpatients may use their own supply if necessary.
