Orders for acetaminophen with codeine (no strength listed) are to be filled with acetaminophen w/codeine 30 mg (Tylenol #3).
Tylenol with Codeine #2 is non-formulary, not stocked.
Orders are to be converted to the liquid preparation at a ratio of 1 tablet (300 mg-15 mg) to 5 mL liquid (120 mg-12 mg/5 mL).
codeine is not to be used in children < 18 years old. Contact MD for alternative therapy.
codeine use should be avoided in breastfeeding mothers due to possible harm to their infants.
