DAPtomycin is classified as formulary, restricted; use for skin and skin structure infections is approved for all prescribers (6 mg/kg once daily dose recommended). Treatment of all other infectious diseases is restricted to ID.
Dose Rounding: Nearest 50 mg increment for patients 18 years and over. Use total body weight in non-obese patients and adjusted body weight in obese patients (obese if TBW/IBW is ≥130% or BMI ≥30).
Recommended AdjBW equation for daptomycin: AdjBW = IBW + 0.4(TBW-IBW)
