Just to let you know the Joint Commission was very impressed with our FormWeb pharmacy site. Although I am still working on it, they especially liked the Sound alike – Look alike section and High Alert section on the page. (and in a later email…) There is a reason why we have retained FormWeb as our formulary website over other vendors. Thanks for re-enforcing my reasoning.

Seven hospitals, 32 primary care clinics, 43 special care clinics, and 26 retail pharmacies have fast access to identical information through FormWeb. It’s easy for the top folks to talk about doing things the same way. It’s harder to get the folks below to actually do it. FormWeb is THE tool for finding all of Fairview’s systemized information and policies.
The physicians are excited about the formulary and the antibiogram. They requested FW become a “Quick Link” on their intranet site, so instead of scrolling down the list on the right of the page, it is front & center (less clicking).
For us, the most useful aspects of FormWeb are:
- Renal dosing,
- Finding formulary alternatives for non-formulary medications, and
- Reviewing policies.

We’re a stand-alone, not for profit, community hospital. We don’t have lots of extra folks to spend time on the FormWeb. But we don’t need that to have a great site.
Once you set it up you don’t spend that much time maintaining it. We spend a maximum of 4 hours a month maintaining. What else can you do for less than 4 hours a month and get this big of a bang?
If I want to revise a policy, I fix it in Word, make a PDF and put the file on the site. It takes me 2 minutes to put it up. I always do it right after I fix the document so I don’t forget. It’s stuff that I do anyway – we just put it on the site.
The most popular item on the site is definitely ORDER SETS. They’ve been phenomenal. The print shop will no longer make them. They’ve been purging them from nursing stations’ files. It’s been a big win for pharmacy.
People in the pharmacy department are so proud of it. The site is gaining us more respect at the hospital.
The sharing of ideas is so helpful and important!! The only thing I hide are medical variances. Everyone has adverse drug reactions – it’s important for the staff to know. It’s OK to show those.
There are 2 other ways FormWeb is extremely important to us:
- The value of having everything in one place – you can enforce your policy because the doc can find the form to fill out for non-formulary
- As a recruiting tool – click on the hospital photo on the homepage and you can get more information about our hospital and our town.
Our CEO visits the site every week to read the news articles I post on the homepage.
I don’t think of myself as a techy guy. I’ve made mistakes, but nothing that the staff at Formulary Productions couldn’t fix. And they never make me feel ashamed.
Working on FormWeb is kind of nice because I feel like I’ve got something done in the middle of a job that goes on and on.

We’re very happy with our FormWeb site.
The biggest way FormWeb has changed life for pharmacists at CMH: they no longer carry 3x5 cards in their pockets or have notebooks to check because they know it’s just a few clicks away. They can toggle between FormWeb and the hospital’s computer system to get most anything they want. They don’t have to call doctors about drug substitutions because docs can look that up themselves.
I really like the way FormWeb helps communicate with nursing. The features nurses use the most are:
- Medications which can be sent home with patients without being relabeled;
- Nurse allowed medication orders; and
- NICU guidelines.
On FormWeb, I think doctors like:
- Drugs in short supply;
- Protocols;
- Shortage recommendation list for substitutions; and
- The non-formulary drug request form.
I also like FormWeb because it saves a lot of money to only carry one kind of ACE inhibitor, for example. Right now I have posted a list of IV antibiotic costs. It was to help make doctors aware of cost differences by putting dollar amounts on drugs.
I’m also proud that the VP of Medical Affairs always highlights FormWeb during orientations for new employees.

The service and support of the FormWeb staff were more than I expected. When I started, our formulary website had not been updated in 4-5 years. I am an administrative specialist for the department of pharmacy in charge of process improvement and training new staff and interns. I thought, “I do not have enough knowledge to take on this responsibility,” but I wanted our formulary website to be presentable and easy for our staff and interns to find information, so I took it upon myself to learn. Clark and Daniel were patient in helping me update our formulary website. They walked me through step by step. They did not make me feel as though I was stupid when I asked simple questions. I still have a lot to learn but knowing they are there to help me whenever my next questions arise is a good feeling.
