Vancomycin Dosing Calculator Background Information:
· Vd is calculated using adjusted body weight, unless patient weighs less than IBW. In that case, the actual body weight is used.
· Vd distribution factor defaults to 0.7 if left blank, otherwise uses the value entered.
· MIC is always 1 for AUC:MIC calculations.
· CrCl is calculated using Cockroft-Gault. This equation uses IBW, unless the patient weighs less than IBW. In that case, the actual body weight is used.
· Loading Doses are calculated using actual body weight. Doses are automatically rounded to the nearest 250 mg that will provide a 25-30 mg/kg load. For ease of administration, doses > 2,000 mg are rounded to either 2,500 mg or 3,000 mg. Loading dose is capped at a max dose of 3,000 mg. · Infusion length is determined according to the dose entered, based on standard rates at Memorial.
· Ke is calculated as follows:
Ke = (0.00083 * CrCl) + 0.0044
· Ke is then used to calculate an expected peak as follows:
