Standardized Titration Order:
25mg QD for 2 weeks, then
50mg QD for 2 weeks, then
100mg QD thereafter. Titration orders outside this regimen will require TLOC approval
Please note, lamotrigine dosages will typically be higher than 100mg QD. Dosages above 100mg do not require TLOC approval, but the starting titrations must be as listed above.
